Chronic Care Management (CCM) is a personalized healthcare service designed to support patients with multiple chronic conditions. The program focuses on improving the coordination, monitoring and overall management of these conditions to enhance the patient's quality of life and reduce hospitalization or other complications.

The First Step
The first step in participation in the CCM program is to become established with a Hillsboro Health Primary Care provider. To schedule a new patient appointment please call 217-532-4351.
Getting Set Up
- At least two diagnosed chronic medical problems will be identified to qualify for the program.
- A referral to the CCM Program will be placed by a Hillsboro Health Primary Care provider.
- A Patient Participation Agreement will be completed.
- The program may last as long as the patient is agreeable.
- The patient may opt out at any time.
Program Costs
CCM is often covered by Medicare and other insurance plans, making it accessible to eligible patients.
What is Included Chronic Care Management Services
- Minimal of 20 minutes of Chronic Care Management services per month.
- Personalized assistance from a dedicated health care professional who will work with you to create your care plan.
- Coordination of care between your Hillsboro Health Primary Care Provider, Specialists, Pharmacy, Laboratory, Radiology, Hospital, Emergency Room, and more.
- Phone check-ins between your visits to help keep you on track.
- 24/7 access to the CCM Care Coordination Team.
- Expert assistance with setting and meeting your health care goals.
