Our family medicine providers welcome patients of all ages, including pediatric patients, sick visits, wellness visits, and chronic condition management. Our primary care providers focus on a variety of different areas such as diabetes, wellness visits, pediatric care, high blood pressure, and more.
We accept many insurances and are often adding more to the list.
Open Monday- Friday, 8 a.m. to 4:30 p.m.
Located in the Douglas-Telfer building at 1220 East Tremont St. in Hillsboro.

Completing the New Patient Questionnaire is easy! Simply download the "New Patient Questionnaire" below, complete it in its entirety and return it to our Primary Care Clinic.
