Illinois law allows patients to appoint someone they trust, such as a family member or close friend, to make treatment decisions should they lose the ability to decide for themselves. This is done by appointing a “healthcare agent” through a Healthcare Proxy Form (link to PDF form).
This law gives patients the ability to make sure that healthcare professionals follow their wishes. An agent can also decide how to apply a patient’s wishes as medical conditions change. Hospitals, physicians and other healthcare providers must follow an agent’s decisions as if they were the patient’s own. A healthcare agent can have as little or as much authority as a patient wants.
